This is an organising suggestion, not clinical advice. Appointments are short, and sleepiness makes recall harder exactly when precision matters most.

Bring specifics rather than adjectives: when sleepiness arrives, what it interrupts, how nights break, what you had to cancel. A fortnight of journal entries is usually more useful than a summary written on the way in.

Write your three most important questions at the top of the page, in case there is only time for three.
